Golf Registration/Sponsorship Form

Thank you for your interest in attending. Currently individual golfer registration is sold-out. Opportunities to play golf are included in two available sponsorships: Long Drive and Closest to the Pin. Of course, we would love to have your support at the fundraising reception which includes dinner, silent auction, live band, and a raffle. Many people come just for the party!

PLEASE NOTE: Due to unforeseen emergencies, we do have individual golfers that are unable to make the golf tournament.
